In 2014, a group of forward-thinking physicians saw an opportunity to bring the medical community together with the purpose of better coordinating medical care and improving the patient experience. These physicians aligned with Gwinnett Medical center to create a coordinated, “clinically integrated” network of physicians to work in partnership with the hospital system. The partnership between physicians and the hospital became even stronger when Gwinnett Medical Center and Northside Hospital merged in 2019, allowing even more physicians to participate in the network.
Sequent Health Physician Partners is the realization of the vision to make quality medical care more convenient and accessible for all patients, provide more personal attention to patients with long-term conditions, and create a greater focus on wellness and staying healthy.

The Sequent Board of Managers is responsible for developing and overseeing implementation of the CIN's mission, vision and strategic plan. The Board provides executive oversight for all CIN operations and final approval for all strategic plans, initiatives, budget, capital expenditure and senior management appointments.
Sequent’s Quality Committee is comprised of physician leaders that provide guidance and oversight for the performance of the network. These physicians help determine the clinical initiatives and the standards by which the network will strive for then work with physicians to achieve those goals. Sequent Health Physician Partners was formally incorporated in 2014 through a cooperative between Gwinnett Health System (GHS) and over 40 physician leaders. Sequent was started to further the GHS mission to "provide quality health services to our community" and provide its citizens with a competitive, integrated and high value health care solution. GHS has committed its ongoing financial and operational support to Sequent Health Physician Partners.
Gwinnett Health System is a 553-bed, not-for-profit health care network that includes two hospitals and other support facilities, more than 4,800 employees, and 800 affiliated physicians. The hospitals system has repeatedly receive national recognition for clinical excellence, ranking in the top five percent in the nation for clinical quality.
Gwinnett Medical Center-Lawrenceville, the system's flagship hospital, provides inpatient and outpatient services as well as 24-hours emergency care, including the Children's Emergency Center, and Level II trauma care. The North Tower features the most advanced technology in health care, and the Strickland Heart Center provides advanced cardiac services, including open-heart surgery. GMC-Lawrenceville's comprehensive array of medical specialty services includes the Center for Orthopedics, Center for Neuroscience and Center for Sleep Disorders. This hospital is also the site of Gwinnett Day Surgery, Gwinnett Extended Care Center, Gwinnett SportsRehab and the Gwinnett Women's Pavilion. The Women's Pavilion offers a Level III neonatal intensive care unit and a high-risk pregnancy unit.
Gwinnett Medical Center-Duluth, located at the intersection of pleasant Hill and Howell Ferry Roads, has 81 beds and expert medical care delivered in a state-of-the-art facility. GMC-Duluth offers 24-hour emergency care and is best known for its Sports Medicine Program, Center for Orthopedics, Concussion Institute, Center for weight Management and Glancy Rehabilitation Center, which offers inpatient and outpatient rehabilitation services for individuals with orthopedic or neurologic conditions.
